(Online Application)- Application Fee (submit $55.00 via credit card with online application or send bank draft with supplemental materials separately)
- Three copies of all Official/Attested transcripts*
and/or non-consolidated marks sheets** of all college work, completed or in progress***, and degree certificates.
- Official test scores sent to CSU, Chico: TOEFL and GRE sent electronically to institution code 4048; Request IELTS and GMAT to be sent directly to University
- Statement of purpose, if required (see program requirements)
- Letters of recommendation, if required (see program requirements)
- Other (see program requirements)
- Financial Support Form and supporting documents
- Copy of Passport
- Copy of I-94 form, if in the U.S.

*Transcripts/mark sheets are considered official if sent directly to the University from the institution attended, or sent in sealed university envelopes with the admission packet. Six complete semesters of course work are required to begin evaluating your application.
**All applicants using a marks-based system of grading must furnish attested non-consolidated marks sheets in addition to any transcripts you may have available.
