Certification of Hepatitis B and Measles and Rubella Immunization is Required |
All first time enrollees who are 18 years of age or younger are required to provide proof of full immunization against the hepatitis B virus. A Certification of Measles and Rubella Immunization is required of all students born after 1-1-57. Immunization Certification forms are available at http://em.csuchico.edu/sro/Forms/ImmunizationCertification.pdf.
